REPORT OF SECURITY INCIDENT

Maintaining the confidentiality, privacy, and security of information in our care is a priority. In March 2024, Human Technology Inc. and Affiliates, became aware of suspicious activity in our computer network. We immediately launched an investigation into the nature and scope of the incident with the assistance of industry leading experts and contacted law enforcement. Through this investigation, we learned that an unauthorized individual gained access to the network without our permission. Files containing individuals’ information may have been viewed or taken by this unauthorized actor. At this point, we do not know specifically what data has been impacted. Human Technology Inc. and Affiliates has engaged leading experts to assist our internal team in learning as much as we can about the data as quickly as possible.

Given the ongoing nature and complexity of the data review, it is likely to take several weeks of continued analysis before enough information will be available to identify and notify impacted individuals. While this comprehensive data analysis is conducted, the company is in communication with law enforcement. This is not an official breach notification. When enough information is available from our ongoing data review, we will do appropriate notifications in the most efficient way possible, as required by law.

Human Technology Inc. and Affiliates is unaware of any misuse of information, however, out of an abundance of caution, and for your support, a dedicated call center has been set up. The call center will open at 8:00 a.m. CT on Monday, April 29, 2024 and can be reached at (866) 528-4805. Please keep in mind that given the ongoing and complexity of the data review, the call center will not be able to provide any details on the identity of the person or impacted data by Monday.

While we are still investigating whether your personal information may have been affected, there are some things you can do to protect yourself:

1. We encourage you to remain vigilant against incidents of identity theft and fraud.
2. Monitor the explanation of benefits statements you receive from your health plan and statements from your health care providers, as well as your bank and credit card statements, credit reports and tax returns, to check for any unfamiliar activity.
3. If you notice any health care services you did not receive listed on an explanation of benefits statement, please contact your health plan or doctor.
4. If you notice any suspicious activity on either your bank or credit card statements or on your tax returns, please immediately contact your financial institution and/or credit card company or relevant agency.
